Application for Graduation

All students graduating from the University of Oklahoma Health Sciences Center must complete an Application for Graduation before their degree can be officially conferred and a diploma issued. Application packets should be picked up at the student's college office, completed, and returned to the Office of Admissions and Records. This packet includes the Application for Graduation, Alumni Card, and Transcript Request forms.

Degree candidates should return the completed packets by:

1. March 1 - Spring Graduates
2. July 1 - Summer Graduates
3. October 1 - Fall Graduates
