Graduation

All students graduating from the University of Oklahoma Health Sciences Center must complete an Application for Graduation before their degree can be officially conferred and a diploma issued.

Beginning Summer 2010, all diploma applications will be filed online using the Student Self Service System. To complete and submit the online application for graduation, login to https://gohsc.ouhsc.edu/ select Academics, My Academics, and Apply for Graduation.       

Degree candidates should complete the online application by:

March 1 – Spring Graduates
July 1 – Summer Graduates
November 1 – Fall Graduates

Transcript Request

Transcripts are free to all current and former students of the University of Oklahoma Health Sciences Center

You may choose from the following options to order an official transcript:

  1. Current students may log into the Student Self Services Gateway: https://gohsc.ouhsc.edu/ 

  2. Mail or fax a signed Request for Official Transcript to Admissions and Records:

  3.  Visit the Office of Admissions and Records at 104 David L. Boren Student Union on the Health Sciences Center campus or 2C11 Tulsa Schusterman Center (photo ID required)

Graduation transcript requests will be processed and mailed as soon as the student’s college posts the degree.

Diploma

Diplomas are available to students approximately 6-8 weeks after their graduation date.

All Bursar accounts and Financial Aid holds must be cleared before the Office of Admissions and Records can release the diploma.

Replacement Diploma

To request a replacement/duplicate diploma, you may contact the Office of Admissions and Records at (405) 271-2359 or by email at admissions@ouhsc.edu to request the Affidavit for a Replacement or Duplicate Diploma.

If your name has changed and you would like your diploma reprinted with your new name, you must submit a copy of the court order documenting your legal name change, the Affidavit for Replacement or Duplicate Diploma, and the $50.00 fee to the Office of Admissions and Records.
