Verification Information

If a student's FAFSA results are selected for a process called verification, the student, and parent, when appropriate, must submit particular income and demographic information and documentation to the Office of Student Financial Aid before the awarding process can occur. Our Office will compare and verify that the information reported on the FAFSA corresponds correctly to tax documents.  Verification is performed to ensure that student eligibility for awards is based on correctly reported information.

If data elements were incorrectly reported, changes to the FAFSA are required to ensure that the student's award eligibility is appropriate.  If a student is selected for verification, the process must be completed before an award package can be presented to the student.  When a student submits the FAFSA, a notice of verification selection is placed on the Student Aid Report (SAR) and our Office will contact the student regarding the documentation required.

IRS Data Retrieval Process

The IRS Data Retrieval Tool (DRT) allows for the automatic retrieval of particular income information when a student and/or parent has filed his/her tax return.  If an individual is eligible for the IRS DRT and chooses to utilize it, the filer's Adjusted Gross Income (AGI), US taxes paid, and certain untaxed income information is retrieved from the IRS and is sent to the school(s) when the FAFSA is submitted.

Tax filers must wait 2 weeks after filing electronically or 8 weeks after filing by paper before they can utilize the IRS DRT, if eligible.  If the FAFSA is filed prior to taxes being filed, a FAFSA correction can be made at a later date.  The user can return to www.fafsa.gov, log in to make a correction, and utilize the IRS DRT.  A new FAFSA transaction will then be sent to the school(s) indicated on the FAFSA.

Applicants will not be eligible to utilize the IRS DRT if:

  • The student, parent, or spouse, as applicable, filed an amended return, or
  • A married independent applicant and spouse filed separate tax returns, or
  • The married parents of a dependent student filed separate tax returns, or
  • An applicant or applicant's parent had a change in marital status after the end of the tax year.

 

IRS Tax Return Transcript

Applicants who choose not to utilize the IRS DRT or are ineligible to use the IRS DRT will be required to request an IRS Tax Return Transcript and provide a copy to our Office.  Copies of signed tax returns will only be accepted in very rare instances. 

Applicants must provide an IRS Tax Return Transcript if:

  • A married independent applicant and spouse filed separate tax returns, or
  • The married parents of a dependent student filed separate tax returns, or
  • An applicant or applicant's parent had a change in marital status after the end of the tax year, or
  • The applicant (or parent of a dependent student) did not use the IRS DRT, either on the initial FAFSA or through the FAFSA correction process, or
  • Information included on the FAFSA using the IRS DRT was changed after being imported from the IRS.

In addition to the IRS Tax Return Transcript, an applicant has the obligation to request an IRS Tax Account Transcript, if an amended return has been filed, and provide both to our Office.  The IRS Tax Return Transcript reflects the original information, while the IRS Tax Account Transcript reflects only the changed elements.

To request an IRS Tax Return Transcript online:

  • Visit www.irs.gov
  • In the Tools section of the home page, click on "Order a Return or Account Transcript"
  • Click on "Order a Transcript"
  • Enter the tax filer's Social Security Number, date of birth, street address, and zip code.  Use the address currently on file with the IRS.  Generally, this will be the address that was listed on the latest tax return filed.  However, if an address change has been completed through the US Postal Service, the IRS may have the updated address on file. For a joint return, use the primary tax filer's SSN, date of birth, street address, and zip code.
  • Click "Continue."
  • In the Type of Transcript field, select "Return Transcript" and in the Tax year, select the appropriate tax year.
  • If successfully validated, tax filers can expect to receive a paper IRS Tax Return Transcript, at the address included in their online request, 5-10 days from the time the online request was successfully transmitted.
  • IRS Tax Return Transcripts requested online cannot be sent directly to a third party by the IRS.

To request an IRS Tax Return Transcript via phone:

  • Call the IRS at 800/908-9946
  • A tax filer must follow prompts to enter his Social Security Number and the numbers in his street address.  Generally, this will be the numbers of the street address that was listed on the latest tax return filed.  However, if an address change has been completed through the US Postal Service, the IRS may have the updated address on file. For a joint return, use the primary tax filer's SSN, date of birth, street address, and zip code.
  • Select Option 2 to request an IRS Tax Return Transcript, then enter the appropriate tax year.
  • If successfully validated, a tax filer can expect to receive a paper IRS Tax Return Transcript, at the address used in the telephone request, 5-10 days from the time the IRS receives the request.
  • IRS Tax Return Transcripts requested online cannot be sent directly to a third party by the IRS.

To request an IRS Tax Return Transcript via paper

  • Visit www.irs.gov
  • In the Forms and Publications section of home page, find IRS Form 4506T-EZ and download the form.
  • Complete lines 1-4, following the instructions on page 2.  Note that line 3 should be the most current address as filed with the IRS.  It is the address where the IRS Tax Return Transcript will be sent.  If the address has recently changed, include the address listed on the latest tax return filed on line 4.  However, if an address change has been completed through the US Postal Service, the IRS may have the updated address on file.
  • Line 5 provides tax filers with the option to have the IRS Tax Return Transcript mailed directly to a third party.  If you wish to have it mailed to our Office, please address it to: Office of Student Financial Aid, 1106 N. Stonewall, Room 301, Oklahoma City, OK, 73117.
    • If a parent of a dependent student requests a Transcript be mailed to our Office, please notify our Office at 405/271-2118.  Otherwise we may have difficulty matching a parent's Transcript with the appropriate student, resulting in a processing delay.
  • One line 6, enter the appropriate tax year for the FAFSA. For example, for the 2013-2014 FAFSA, 2012 is the tax year.
  • The tax filer, or spouse, if requesting information from a joint tax return, must sign and date the form and enter the telephone number.  Only one signature is required to request a Transcript for a joint return.
  • Mail or fax the completed request to the appropriate address or number listed on page 2.
  • A tax filer can expect to receive his transcript within 5-10 days from the time the IRS receives and processes the signed request.
    • Processing form 4506T-EZ means verifying/validating the information provided on the form.  If any information does not match the IRS records, the IRS will notify the tax filer that it was not able to provide the Transcript

High School Completion Sheet

  • Some students will be required to verify high school completion status.  This page is used only for the student's reference, no signature is required. One of the forms of documentation listed on the sheet will be required to be returned to the Office of Student Financial Aid.

Identity and Statement of Educational Purpose for Independent Students

  • A student selected for this portion of verification will be required to verify his identify and sign a statement that certifies that any federal aid he receives will be used only for education purposes and to pay for attending OUHSC.
  • The student required to fulfill this requirement will be required to appear at the Office of Student Financial Aid in person to complete this form.  If the student cannot appear in person, the student will need to complete this form in the presence of a notary.

Identify and Statement of Educational Purpose for Dependent Students

  • A student selected for this portion of verification will be required to verify his identify and sign a statement that certifies that any federal aid he receives will be used only for education purposes and to pay for attending OUHSC.
  • The student required to fulfill this requirement will be required to appear at the Office of Student Financial Aid in person to complete this form.  One parent will need to sign the second page, but does not have to appear in person.  If the student cannot appear in person, the student will need to complete this form in the presence of a notary.

  • This worksheet is used to verify particular income data elements reported on the FAFSA as listed by a student, and spouse, if applicable.  If you are eligible and would prefer to utilize the IRS Data Retrieval Tool (DRT), please see the details above.  If our Office receives your corrected FAFSA results with a successful Data Retrieval, you will not be required to complete the worksheet.
  • If you do not, or cannot, utilize the IRS DRT, you will need to complete, sign, and return the worksheet and provide to our Office copies of all W-2s, 1099s, and/or Schedule Cs, as appropriate.
