Area 1: The bulk of the lesion is composed of intertwining swollen collagenous fibers of variable dimension. The key feature for diagnosis are the elastic material, in the shape of a cluster of beads (arrow) with staining intensity almost identical to that of the collagen fiber.

History: This specimen was removed from the neck of an elderly woman.

 

Histologic Highlights of this Case:

  • Elastofibroma is a fibroelastic pseudotumor.

  • Histologically, it is usually poorly defined and it is composed of a mixture of intertwining swollen collagenous fibers admixed with a more or less equal portion of elastic fibers. On histologic sections, the elastic fibers appears as clusters of beads with some of the demonstrating a radiating pattern. The elastic material is best demonstrated on elastic stain. Many of these collections of elastic materials are reminiscent of flowers. On longitudinal view, the elastic fibers can be branched or unbranched.

  • A few spindle cells are present and they are typically negative for S-100.

  • Nuchal-type fibroma also contains haphazardly arranged thick collagen fibers that can be mistaken as elastofibroma. However, Nuchal-type fibroma doe not contain the elastic materials. Many cases are associated with diabetes-mellitus and they are more common in man.
