| Hiring Freeze Form and Guidelines |
Effective 1, 2008. This form must be completed and returned to Human Resources (HR) before any employment listings can be authorized. Please contact your college Faculty Affairs personnel for additional guidelines. |
| Nepotism Policy and Management Plan / Waiver Request Form | The University recognizes that there is an inherent conflict of interest when an employee in an administrative or executive position is directly responsible for making hiring, promotion, salary, or tenure decisions about a family member in that same budgetary unit. It is the responsibility of the budget unit to seek a waiver before offering employment to any person whose employment without a waiver would violate the Nepotism Policy. |
| Notice of Vacant Position (NOV) | This form is used to open a new faculty position and to begin the recruitment process. Please contact the Equal Opportunity Office for additional information regarding recruitment procedures to ensure compliance with University and Federal guidelines. |
| Affirmative Action Summary | This form is submitted when the candidate has been selected at the conclusion of the new faculty recruitment process. Please contact the Equal Opportunity Office to determine if procedures for a national search were satisfactorily followed before completing. |
| Employee Personal Data Form | All new HSC employees must complete this form. Only employees who hold a teaching or research appointment with the University of Oklahoma will need to complete page two (Education) of the form. This form must be included with the ePAF attachments. |
| Faculty Information Form (Optional) | This form can be used by colleges for the sole purpose of obtaining information required for licensure, board certifications, and their internal database. This form is not required by Human Resources. |
| 5050A (Optional) | This form can be used by departments/ colleges for academic volunteer actions only. This form should not be used however in place of ePAF for paid academic actions. |
| Loyalty Oath | This document is required by the State of Oklahoma. This document must be notarized and returned before the employee can be issued a paycheck. This form must be included with the ePAF attachments. |
| Employment Eligibility Verification (I-9) | This is a federally mandated employment eligibility verification document. This completed form and the original documents providing your employment eligibility must be included with the ePAF attachments on or before your the working day. |

| Sabbatical Leave of Absence | A sabbatical is to provide opportunity for continued professional growth and new or renewed intellectual achievement. Sabbaticals can be for a period of 6 months or 1 year. See Section 3.12 in the HSC Faculty Handbook for the entire policy and procedures. |

| Recommendations for Tenure and Promotion (blank) | These forms include the tenure/promotion dossier cover sheet which provides a useful summary of data concerning the candidate and a summary of tenure/promotion recommendations made; and the recommendations of the dean and chair. See Section 3.9 (tenure) and 3.11 (promotion) of the HSC Faculty Handbook. |
