This document is the Final Report of the Child Welfare Review Committee for the Death and Near Death of Children with Disabilities.

The Review Committee was created in response to the deaths caused by abuse or neglect of two teenaged children with significant disabilities. In both cases the system or systems with a responsibility to provide services to these children failed.
