Patient Services

The Oklahoma Diabetes Center offers many treatment and education opportunities for patients who are concerned with preventing or controlling Diabetes.  The Oklahoma Diabetes Center is an American Diabetes Association Nationally Recognized Diabetes Education Program Provider.  We provide group and individual classes which are taught by highly trained specialists in diabetes.  Additional services are available for complete management of Diabetes.

• Multiple Daily Injections
• Individual Appointment to Begin Pump Therapy

In addition to Diabetes, the Center treats endocrinologic disorders, including thyroid, adrenal, pituitary, growth, gonad, metabolism, and disorders of sexual development.

Physician Referral Form
The Physician referral form is required to obtain service reimbursement from insurance companies.  Please print out this form, and have your physician's office fill out the information and fax it back to us at (918) 619-4801.
